IMPORTANT FORMS TO COMPLETE
Parents please complete ALL THREE of the following forms for your child to be able to attend camp.
CHECK IN IS AT 6:00AM ON JUNE 28 AT CHETS CREEK HODGES
Please complete the Participant Waiver for FUGE Camps. This will include Track Time selections for your student, so please consult them prior to completing the form. You can learn more about Track Times offered this year at here.

Please complete the Recreation Waiver for our camp venue, Ridgecrest Conference Center.
Ridgecrest Waiver
Please complete the Medical Waiver for Chets Creek Church at the link below. Students who are 18 years or older should complete the waiver for themselves.

Travel Accommodations
If your student needs a travel accommodation because they will have to leave early or come late to FUGE Camp, please carefully read and complete this Travel Request Form no later than June 14, 2026.
